Coming soon to iPhone in the UK

Less to hold in your head.

Coming Home: Family Care brings the hospital discharge, the medicines, the daily "I'm OK", the letters and the help you might be entitled to into one place. Shared with the people who help, so no one is doing it all alone.

Free to join, no card needed. Founding members get 3 months of Premium free at launch.

The Today view for Sarah: Mum checked in at 9:12, a note that Mum walked less than usual this week with the numbers, and today's tasks.
The I'm OK view for Margaret: a large I'm OK today button, with options to ask for a call or a hand with something.
Why we're building this

Coming home from hospital is where the organising starts

Discharge can happen fast. Then the family is left holding the medicine changes, the visits, the letters and the worry, often from miles away.

Get ready for them coming home

Questions to ask the ward, a home-readiness list and a shared 30-day plan, so the first weeks at home feel organised, not rushed.

Share the caring, not just the worry

One circle for the family. Everyone can see who is visiting, who is collecting the prescription and what's been done. Family join free, always.

A gentle "I'm OK" every morning

One big button for Mum or Dad. If they haven't checked in by the time you choose, the family gets a message in the order you set. A daily hello, not an emergency service.

Sarah lives 90 minutes from her dad and works full time. She found out he was coming home from hospital the same morning. By lunchtime she was ringing the ward, the pharmacy and her brother, and keeping it all on the back of an envelope.

Sarah is an illustrative example, not a real person or a review. Coming Home is built by people who have run home care and seen this moment many times.

It's not just Sarah

What Coming Home does

Everything in one place, for everyone who helps

Practical tools that take work away, not add to it. Built for the person organising, the person being cared for and the rest of the family.

Coming home from hospital

Bring them home with a plan

  • Questions to ask the ward before they leave
  • A home-readiness list: access, bathroom, bed, heating, food and equipment
  • A 30-day family plan, with tasks shared across the circle
  • A note of what's changed: new, stopped or changed medicines and routines
  • Your rights as a carer, with a drafted request for a carer's assessment
Mum's first 30 days: day 3 of 30, settling in well, with ward questions, home set-up and medicine changes ticked off.
Care circle

Everyone on the same page

  • A shared calendar, tasks with names on them and an updates feed
  • Key contacts in one place: GP, pharmacy, district nurse, neighbours
  • Invite as many family and friends as you like. They join free, always
  • One subscription covers the whole circle, so there's no argument about who pays
Mum's circle: Jo is bringing shopping on Saturday, Tom picked up the repeat prescription and Margaret checked in feeling brighter.
Medicines

Know whether Dad's had his tablets

  • A medicines list and reminders, for them and for you
  • A shared record of each dose: taken, given, missed or not needed, and who recorded it
  • Tablets left and repeat prescription reminders
  • A medicines record to show the GP or pharmacist

Coming Home records what's on the label. It doesn't give advice about medicines. Questions? Ask your pharmacist.

Dad's medicines today: each dose with its time and whether it was taken or given, plus tablets left for each medicine.
Letters

Understand any letter in plain English

  • Scan an NHS, hospital, council or DWP letter
  • Get a plain English summary, key dates, who to contact and a to-do list
  • Dates go straight into the family calendar
  • The original is kept alongside, so nothing is lost

If anything is unclear, call the number on the letter.

A clinic appointment letter in easy read: when, where and who to contact, with to-dos like bringing Mum's medicines list.
Help and money

See what help you might be entitled to

  • A 3-minute entitlements guide: Attendance Allowance, Carer's Allowance, Council Tax reductions, carer's assessment, Disabled Facilities Grant and more
  • We help you draft your answers. You read every one and send the form yourself
  • A Lasting Power of Attorney list, explained simply

Guidance only. DWP and your council make the decision.

What you may be able to claim: 5 things to look into, including Attendance Allowance, Carer's Allowance and a carer's assessment.

Privacy

You choose what you share

The person being cared for owns their information. Coming Home is built so they stay in control, and so the family is never locked out of what's been shared with them.

  • They decide, type by type

Steps, sleep, check-ins, medicines: each one is shared only with the people they pick. They can change their mind at any time.

  • Stop sharing straight away

Turning sharing off takes effect at once. Nothing is shared without their say-so.

  • Never sold, no adverts

No ads and no ad trackers, and we never sell your information. Apple Health data is never used for marketing.

  • Download or delete it all

Export everything as a PDF or CSV, or delete your account and data, free, whenever you like.

Before we start: your circle sees what you choose, health data stays on your terms, and you can download or delete everything.

Questions

Questions families ask

When will Coming Home be available?

We're building it now and aiming to launch on iPhone in the UK in spring 2027. Dates can move. Waitlist members will hear first.

Is it only for iPhone?

It starts on iPhone, with an I'm OK button on Apple Watch. Android is planned for later. With Premium, family members without the app can still get I'm OK messages by text.

Does the person I look after have to use it?

No, but it works best when they do. They can tap I'm OK each day and choose what to share. You can start on your own and invite them when they're ready.

Is Coming Home a medical device, or a replacement for the GP?

No. Coming Home helps families organise everyday care and shows information as it was recorded. It doesn't give medical advice and it isn't an emergency service.

If you're worried about their health, contact their GP or NHS 111. In an emergency call 999.

What does "different from usual" mean?

If the person chooses to share Apple Health data, Coming Home can compare it with their own usual pattern and let you know when something looks different, for example fewer steps than usual this week. It shows the numbers so you can see why. It never names a condition. It's simply a nudge that you might want to check in. This is part of Premium.

What is the founding member offer?

If you join the waitlist before launch, you'll get 3 months of Premium free when the app arrives. No card is needed now. We'll remind you before it ends, and you can carry on with the free plan at any time.

Can one subscription cover both parents?

One Premium subscription covers one person being cared for, and everyone in their circle. We're planning a family plan for up to 3 people, and we'll share the price before launch.

What happens to my waitlist details?

We keep your email and answers to tell you about the launch and to understand what families need. This website uses no cookies and no ad trackers. The privacy notice explains more, including how to ask us to delete your details.

Who is behind Coming Home?

A small, independent UK team, built by people who have run home care. We're talking to carers now, and your answers on the waitlist shape what we build first.
